The RIDDOR deadline, counted for you. 10 days for a death, a specified injury or a dangerous occurrence; 15 days for an over-seven-day injury - both counted from the date of the incident.
Investigation and root cause. Assign an investigator and work through root cause analysis, 5 Whys, a fishbone, a fault tree or a bowtie. Each recommendation becomes a corrective action with an owner.
Closed only when it is done. An incident cannot be closed until its root-cause analysis and the notifications it requires are recorded.
